Abstract

Mit Hilfe von Modellen der neoliberal ausgerichteten Betriebswirtschaft, des “klassischen“ Managements sowie der Organisationsentwicklung soll Qualität entwickelt, gemessen und evaluiert werden. Diese Entwicklung hat zu einer Formalisierung, Technisierung und “Maschinisierung“ der Pflege geführt. Für Pflegeheime gilt: Qualitätsvorgaben sind differenzierter geworden, und eine Intensivierung des Qualitätsmanagements ist erkennbar. Die Frage stellt sich, welche Konsequenzen ein lineares, funktional und zweckrational ausgerichtetes Qualitätsverständnis hat, welches durch die Reform der Pflegeversicherung weiter akzentuiert wird. Der Hauptkritikpunkt besteht darin, dass durch das vorherrschende Qualitätsparadigma der Pflege eine abhängige und funktionale Rolle zugeschrieben wird und substantielle Fragen der Qualitätssicherung ausgeblendet werden.

Quality of Care and the 2008 Nursing-Care Reform in Germany

By drawing on models of (neo-)liberal applied economics, ‘classical’ management, organization development and the progression of staff, it has been possible to develop, measure and evaluate quality. The result of this development has been the formalization, “technologization“ and “mechanization“ of care. For nursing homes, it is the case that quality standards have become more and more differentiated, and that quality management has become stricter and more intensive. This raises the issue of the possible consequences of a functional and linear conceptualization of quality that is based upon an instrumental form of rationality. This conceptualization has been accentuated yet further by the reform of nursing-care insurance. The main critical point is that the mainstream quality paradigm reduces, and forces, nursing into a dependent and functional role. This ignores substantial questions of quality.
