Abstract
Welcher Wettbewerb ist im Gesundheitswesen der richtige? Hilft es dem Patienten und der Krankenkasse, wenn eine Einzelleistung durch den Wettbewerb um 20% billiger wird, aber diese Leistung gar keinen Gesundheitsnutzen produziert oder, noch schlimmer, die Anzahl der durchgeführten Leistungen ohne gleichzeitigen Nutzen um 20% steigt? Die Autoren plädieren vehement für eine Neuausrichtung der Anreize und eine Honorierung, die statt des Einzelpreises den geschaffenen Gesundheitsnutzen und damit die Ergebnisqualität für den Einzelnen wie für die Versichertengemeinschaft in das Zentrum des Wettbewerbs rückt. Eine solche Honorierung – so die Argumentation der Verfasser – benötigt regional organisierte Systemlösungen als Treiber von Effizienz und Effektivität und Partner der Krankenkassen.
What kind of competition is the one to choose for health care systems? Is it helpful if competition reduces the fees for certain services by 20% even if there is no extra health benefits from those services or, which is even worse, the number of actual treatments under those services increases by 20% for no corresponding increase in benefits? The authors propose a change to the current payment scheme with a realignment of incentives. This change would enable competition to focus on the health outcomes and benefits for patients and populations rather than on prices for single services. The authors argue strongly in favour of the need for carefully designed regional systems, firstly, to drive efficiency and effectiveness as the organising structure for this goal and, secondly, to act as partners for the health-care funds.
